Bryan Sollazzo, male. Positive smear indicating sexual transmitted disease,Hep C (subjective indication of needle sharing) Herpes [(subjective indication from unprotected sexual intercourse with multiple female/male partners) time length of unprotected sexual intercourse dating backc to 6 months to the date], patient is compliant with medication and treatments, stated that he has taken Valtrex when only having a "break out" from Herpes (x2) Patient has willingly stated he wanted to have an HIV/AIDS test, results are to be determined ( 16 sexual partners have been contacted by state officials for consent). Patient has been refered to therapy for psychatric disorder, which have been determined (bipolar disorder II, antisocial disorder and PTSD ( determined from drug and alchol relapses). Antabuse has been subscribed (patient is non compliant). Family history is non exsistent, patient verbalized "my family said i am dead to them when they found out i was a heroin addict" . Patient has agreed to seek help in psychatric ward and attend alchol/ drug classes in order to stay out of jail (court date is pending for domestic violence and hyperdermic needle possession, relased on parole #bryansollazzo #hepatitisc #wilmington #massachusetts
